Duties:


  • Establishes and maintains relationships with referral sources, hospitals, physicians and Case Managers.
  • Educates the medical community of clinical programs that improve patients’ outcomes and allow them to maintain in their home environment.
  • Participates in developing new business opportunities through community outreach.
  • Presents disease-specific clinical programs to promote positive patient outcomes.
  • Works closely with the clinical and operation team to insure seamless care transition coordination from an acute care setting to home.
  • Participates in on-going assessment of community needs Assists in development of agency-wide marketing plan.

Skills/Requirements:


  • OND, HND/BSc Degree in any discipline but have a passion for helping people
  • Ability to work under minimal supervision
  • Must have team spirit
  • Excellent verbal and written communication skills.
