• A Copy of this Exam slip.
• A duly completed accreditation form
• Photocopy of Identification (Drivers Licence, National ID Card, International Passport or Local Government Certificate of Origin with an attached passport photograph that is duly stamped)
• One (1) Number Recent Passport Photograph.
• Any other document that will assist the examiners to quickly identify you.

You are expected to arrive early at the test center as verification of credentials will take place before the test begins.
