we paid 90k 4 1st yr & 50k 4 2nd yr.forms are in d skul nt online

the form is #10,000.00, once u get 2 futa go 2 centre for continuing education CCE where u pick bank teller 4 payment 2 wema bank, after payment go back 2 CCE 2 collect ur form. NOTE: elect as 2 undergo screening test which will comes up on 22nd of sept 2012.
