Ui pg form- master

Please i want to find out if ui pg form is still available and how do i get it in lagos.
Please ur information will be highly honoured. I have been tring to get the info but all in no avail.
I have gone through their site and i try to register/pre-registration but it wasnt going at all.
I need to obtained the form this year. Thanks

Use internet explore. D site does nt work wt firefox. Hope dat will solve it

@soul4love....thanks but I hv a problem n dats d final submittion or arent there any? If their is I hvnt seen it. I mine nt suppose to print out my form? No column for dat too. thanks anticipate ur reply.
