please guys how do i get this following documents? do i have to travel down to the east to get them? Parents/Guardian Consent Form, Local Government Attestation Card, Acknowledgment Form. pls help. and which of the banks are we meant to pay? whoever has successfully submitted should kindly put me through!

U will print those documents from their(navy) website. As 4 d payment u can go 2 any of these banks:access,fidelity,sterling and uba. Hope this will help?

@Vprince, Yes, for the Parents/Guardian Consent Form, your parents will have to sign, that mean, anywhere they are Ʊ will have to get to them to sign it, while for the Local Government Attestation Card, Ʊ will have to go your local Government chairman or the secretary to attest the form for Ʊ!
So be fast about it

Richest man i'm in deep shit right now! Im presently in lagos! Do i have to start travelling down to the east just to get the local govt form signed? What do i do?

Were you able to download those documents from their site? If yes pls guide me on how to do it because I ve been finding it uneasy to download the forms.

Alternatively, can i take the forms to my state liason office here in lagos for attestation? Pls guys respond let me know exactly what to do! I can imagine travelling down to the east. I mailed them now and they were telling me trash to adhear strictly to the rules.

@vprince u can sign d local govt form in ur state liason office here in lag.No need of traveling 2 d east 4 it.

Of course u can. That's part wat d liason offices ar there for. Just locate ur state's liason office in lag here,explain 2 d officers in charge,they wil sure sign it 4 u.

Richestman it seems the parents concent form and LGA form are not ready for download yet. Or is it that I do not know how to do it? Guy pls shed light. I need the forms to travel to my LGA. Guys any body with useful info should help cos am stucked right now. I will really appreciate.

I'll get back to u. Guys i learnt the questions are drawn from gmat and general knowledge. So richest man, if i get u right, i have to start up with the application first before going to the bank? bt ds ppl are nt treatn us fair. At d end, they take 4 or 5 ppl per state

Very urgent. Checked my application status and saw APPLICATION SUCCESSFULLY SUBMITED. but the consent forms wasnt clickable. Are my suppose to print out the consent cards before checking my application status? And did anyone recieved a confirmation sms or email after submitting? Pls reply asap! Or do i have to re apply again? I'm stuck.

@Vprince. U cn check d last tab on dat aplication stuf. It wil pop up a tab where u wil click 2 put ur id, ref num n validation num and then d forms. U print them out. Then make sure theres no pop up window.Hope dis help.

Click (or touch d mouse) on d Print consent form. It wil show under it d folowing:parents or guardian form,lga form, acknowledgement card. U then click on each one. It wil lata show u (on top of it)"ensure that theres no pop up windows". Click on d options that is at d right side of d window(u wil find it immediately after that Id box) , choose d last option. D form wil open in another window. U print it.
