Health Personnels/Facilitators required
Duties:
- Establishes and maintains relationships with referral sources, hospitals, physicians and Case Managers.
- Educates the medical community of clinical programs that improve patients’ outcomes and allow them to maintain in their home environment.
- Participates in developing new business opportunities through community outreach.
- Presents disease-specific clinical programs to promote positive patient outcomes.
- Works closely with the clinical and operation team to insure seamless care transition coordination from an acute care setting to home.
- Participates in on-going assessment of community needs Assists in development of agency-wide marketing plan.
Skills/Requirements:
- OND, HND/BSc Degree in any discipline but have a passion for helping people
- Ability to work under minimal supervision
- Must have team spirit
- Excellent verbal and written communication skills.
