@johnpaul its not compulsory to collect d form in ur state of origin in d headoffice in LAGOS AT OJODU the form is been given accordin to ur state of origin, lagos, abia n adamawa were slated for collection today, married pple r not allowed, any form of sight problem or usin glasses is not allowed n some other conditions before ur #1000 will be considered. Cheers to all who will be applying me na average height

For those who bought the form do they request the letter of identification before issuing the form or when you are submitting the form. pls help!!!

Please take note that the form is now #500 only, I just got mine through my brother inlaw who is one of thier commandant and every other person got theirs at the same price. Before you can get the form you will pass through different screening exercise, you must be tall and your foot must not be flat.

Hi house, for those of u dat applied, is it compulsory i need a drivers' license and do i have to go to my State capital or any of their office near me. Because my residence is very far from the capital. Thanks
@faithlarge, drivers lic is not compulsory and u can buy d form @any of dia ofis for #500.afta mitin som of dia instant reqmt like height,no flat foot etc
